Welcome Families!
It is time to register your children for our 2010-2011Faith Formation programs! Registration will take place during the week of August 2- August 8 in the Faith Formation Office. We will accept registration based on the schedule below. Please plan on spending a few minutes with a member of our Faith Formation staff for completion of your family’s registration. If you did not receive a registration packet in the mail, please print the registration forms listed on this page, including the Registration Cover Letter, Faith Formation Overview, and Family Volunteer Form . You will also need to print and complete the following:
- Preschool - 6th Grade: Registration Form and Emergency Card for each child
- 7th - 8th Grade: Registration Form, Rules & Model Release, and Diocese of Sacramento Youth Form for each youth
- High School: Registration Form and Diocese of Sacramento Youth Form for each tee

If you have children in multiple grade levels, we will process registration for all family members at the first applicable registration session. (For example, if you have a fourth grader requesting the Monday session, a seventh grader and a Kindergartner, we will register all three on Monday, August 2, 2010.)
For those unable to attend the appropriate registration sessions listed above, we will hold an additional registration session for all grade levels as follows:

Based on the above prescribed schedule, all registrations will be processed on a first-come, first-served basis.
If it is impossible for you to attend any of the registration sessions listed above, please call the Faith Formation office at (916) 624-5827, ext. 213 prior to July 26th to make a registration appointment. Mail in registrations will not be accepted. Please bring ONLY your own family’s registration forms.
**All volunteer Catechists, Aides and CORE Team Members that have made a weekly commitment to the Faith Formation programs will be pre-registered prior to Monday, August 2. Please give careful consideration to the possibility of being a volunteer catechist, aide, or core team member for the children.
Our fees for the 2010-2011 school year will be as follows:
High School, Preschool or Kindergarten fees: $ 50.00
Elementary and Middle School fees:
1 child $ 75.00
2 children $100.00
3 or more children $125.00 (Maximum yearly fee to be paid by a family.)
Lead catechists and/or core team members who make a weekly commitment to our program are exempt from paying fees, and weekly aides will receive a 50% fee waiver. As always, financial circumstances will not interfere with a child’s participation in the program. If paying the fees presents a hardship, please mention this at the time of registration and other arrangements can be made.
